Create an Issue (Mobile)

An Issue is a recorded problem. On mobile you can create one in about 60 seconds — tap the FAB, describe what you're seeing, take a photo, and submit.

Also available on web: Create an Issue (Web)

✅ At least one photoEvidence is requiredYour phone camera
✅ An Asset (optional but recommended)Makes history and reporting much more usefulAsk your org admin or use the QR scanner to find the asset

Step-by-step

1. Open the create form

Tap the blue + FAB → Create Issue.

(Or tap Issues in the drawer menu, then tap the create button in the top right.)

2. Select a location anchor

You're asked to specify where the problem is. You have three options — pick the most specific one you can:

  • A specific asset — search by name or tag number, or tap the QR icon to scan the asset's code. This is the best option. It ties the issue to exact equipment history.
  • A space — tap a space button (e.g. Walk-in Cooler #1) if you know the area but not the specific unit.
  • Whole store — the default if you skip both above. Use this only when the problem can't be attributed to a location.

The filing anchor at the bottom of the form updates to show what you've selected: "Filing against: [Asset › Space]" or "Filing against: [Store]".

3. Issue type

Type a short description of the problem. Examples:

  • Temperature drift
  • Compressor not cycling
  • Leak at condenser
  • Circuit breaker tripping

4. Description (optional)

Add more detail in the multiline field — what you observed, when, how serious it seems. Example: "RTU-3 making grinding sound since this morning. Zone 2 ambient is 6°F above setpoint."

5. Severity (required)

Tap one of four levels:

SeverityUse when…
LowMinor problem, not affecting operations
MediumNoticeable problem, monitor closely (default)
HighAffecting operations or product safety
CriticalImmediate threat to safety, compliance, or major loss

6. Due date (optional)

Tap Pick a due date to open the calendar. Set a date if this needs to be resolved by a specific time.

7. Add evidence photos (required — min 1, max 5)

Tap Take photo to use the camera directly, or Library to pick from existing photos.

  • The photo count shows as Evidence photos (N) at the top of the section.
  • Each photo shows a thumbnail with a × button to remove it.
  • You can add up to 5 photos per issue.

Tip: Take a wide shot showing the equipment in context, then a close-up of the specific problem area.

8. Submit

Tap Submit issue. The button shows a loading indicator while the photos upload and the issue is created.

On success you'll see a brief confirmation message and return to the Issues list, which now shows your new issue at the top with status Open.

Adding more photos after creating an issue

  1. Open the issue from the Issues list.
  2. Scroll to the Evidence section.
  3. Tap Add evidence and take or select additional photos.
